Address Change Notice

You were mailed an Address Change Notice because the Supervisor of Elections received information that your legal residence has changed. Your voter registration record has been updated to the new address, which is printed on the card. If the new address printed on the card is incorrect, please complete the form below. If you use this online form, you do not need to also mail the notice back to our office.

If you have changed your name, please click here to be taken to the FL Voter Registration Form. This form must be completed and returned to our office in order to process a name change.

 

MOVED OUT OF FLORIDA?

  • If you no longer live in Florida and wish to cancel your Florida Voter Registration, please STOP HERE! Check the appropriate box on the card you were mailed, sign and return the card to our office.

Please use this form if you have received an Address Confirmation Request, an Address Change Notice or an Address Confirmation Final Notice from our office and you wish to confirm you have NOT moved out of the county.

You can use this form if both of the following statements are true.

  • Your name is correct on the notice
  • You continue to maintain your legal residence in this county

Otherwise, if you have changed your name or you have moved elsewhere within the state of Florida, you can not use this form and instead must use the official Florida Voter Registration Form to update your registration information.

Finally, if you have moved your legal residence outside the state of Florida and wish to have your Florida voter registration cancelled, please sign and return the confirmation notice that you received.
